Ending the map (going back to the main menu)

Hi, I've got this issue for a while now and I'm pretty sure it's my fault in using the wrong entity's or something.

What I have right now is a trigger_once inside the ending lift instance, which triggers on StartTouch a fade to black and then triggers a game_end to do "EndGame" a second after.
When I go in the lift in-game it just fades to black and then I stay there in the non-moving lift (can hear myself jumping, etc).

I have no idea where to look for a referance because Valve probably changes maps and not goes back to the menu.
How do I fix this?
