One suggestion: I would let the trigger_hurt do 1000 damage or similar. That way, players would die as soon as they fall in the water. Right now, they die so slowly that there is time for two explosions. I think it would be better to just have them respawn asap since it's clear they are dead anyway.
In the first chapter, there are some exploits possible that Mumbles found when we were playing it. But they should be easy to fix:
